Vitamin B12 deficiency mimicking neuroimaging features of motor neuron disease.

چکیده
A 45-year-old man presented with 2-month-history of progressive gait disturbances and behavioral changes. Examination showed vibration sense compromise and pyramidal signs of release. Blood test revealed low serum vitamin B12 (120 pg/mL; normal range>200 pg/mL). Brain MRI disclosed hyperintense signal in corticospinal tracts, a similar pattern observed in motor neuron disease (MND) (Figure). Replacement therapy was started and there were motor and cognitive improvements. Vitamin B12 deficiency may present with different neurological syndromes1. Encephalopathy related to B12 deficiency presents with nonspecific white matter changes2. Although MTC brain MRI sequence was normal, our patient presented neuroimaging features observed in MND3.

Subacute combined degeneration : report of two cases
Subacute combined degeneration ( SCD) due to vitamin B12 deficiency is common in North of Europe but sometimes it is seen in Iran. It can develop before the symptoms of anemia is obvious, with myelin injury and then axon and even neuron destruction in spinal cord, brain, optic and peripheral nerves. In this study two cases are reported; in the first patient, this disease in manifested by vitami...

INTRODUCTION Amyotrophic lateral sclerosis (the most common form of motor neuron disease) is a progressive and devastating disease involving both lower and upper motor neurons, typically following a relentless path towards death. Given the gravity of this diagnosis, all efforts must be made by the clinician to exclude alternative and more treatable entities. Frequent serology testing involves s...

Introduction: Megaloblasc anemia is an uncommon problem in childhood most frequently associated with vitamin deficiency or gastrointesnal disease. The common causes of megaloblasc anemia are vitamin B12 (cobalamin) deficiency and folic acid deficiency. Familial selecve malabsorpon of vitamin B12 associated with proteinuria firstly was described by Imerslund (1960) and Grasbeck et al (1960)...
متن کاملVitamin B12 deficiency: Cognitive impairment and neuroimaging correlates
Vitamin B12 deficiency has been associated with various neuropsychiatric symptoms, and to be a reversible cause of dementia. As the negative impact of low vitamin B12 status on cognition can range from subclinical neuronal metabolic derangement to an overt debilitating state in line with permanent brain structural changes, prompt recognition of vitamin B12 deficiency is of paramount clinical im...
متن کاملEffects of Vitamin B12 in Neonates and Young Infants
Vitamin B12 (cobalamin) is an essential coenzyme for nucleic acid synthesis. Animal protein is the major dietary source of vitamin B12. Deficiency of vitamin B12 leads to megaloblastic anemia, degeneration of the brain, spinal cord, peripheral nerves, and abnormalities of epithelial tissues. Two factors are necessary for the cure of megaloblastic anemia: one in food (extrinsic factor) and one i...
